Visits with Cityblock are flexible. They can take place by phone or video call, at one of our Hub clinics, or even in your home.

Care when you need it

For urgent care after hours or on weekends, call our 24/7 care line. You’ll be connected with a licensed provider in minutes. Depending on time of day, we may also be able to come to you and provide care in your home (where permitted by law).

Primary care provider vs non-primary care provider

All Cityblock members get the primary care services discussed on this page. The only difference is how you get them.

If Cityblock is your PCP, you get these services directly from us:

  • Your Cityblock Community Health Partner, PCP, care team, and external providers or specialists will all work together with you. 
  • They’ll help you navigate the healthcare system, schedule appointments, follow up to make sure you know what to do next, and more.
  • If you need care after hours or on weekends, you can call our 24/7 care line to connect with a licensed provider.

If Cityblock is not your PCP, we’ll work with your existing doctors to get you the care you need.

  • Your Cityblock Community Health Partner and care team will work together with you to support your health needs. 
  • We will work with your PCP to make sure all your providers are on the same page.
  • When you need primary care services, you can get in touch directly with your PCP. But if you need help or have questions, you can also call Cityblock for support scheduling appointments and meeting your medical needs.
  • If you need care after hours or on weekends, you can call our 24/7 care line to connect with a licensed provider.
