In-home urgent care*
Get 24/7 virtual care without leaving your home and if you need urgent care services we’ll send a team to your front door*.
Primary care
Get primary care that’s centered around you. We’ll get answers to your health questions and check in on you if and when you need us.
Mental health
Your mental health is just as important as your physical health. If you're dealing with stress and anxiety, need support for substance use disorder, or if you need someone to talk to, we're here.
Social care services
We want our members to thrive between doctor visits. That’s why we connect them with community resources for help with necessities like housing, employment, legal aid, food, child care and more.
Care coordination
We put members in the driver’s seat! We’ll help manage appointments with doctors, specialists, and case managers to make sure nothing falls through the cracks.
24/7 care line
Your health doesn’t have business hours, so neither do we. Our members have a direct line to a provider for medical advice, 24/7.

Members are always welcome to call us with any questions or concerns they may have. Our care teams are ready to give one-on-one support to explain how Cityblock works, help figure out our services, and point members in the right direction.
