How we work
Cityblock is here to fill gaps in your care. All the things you know would make you healthier and happier, but might not be part of regular medical services. Things like medical advice and treatment when the doctor’s office is closed. Support making diet or lifestyle changes. And much more.
Medical care
Depending on your plan and preference, we coordinate with your existing providers, or assign doctors based on your specific needs.
In-home urgent care*
We can treat you for both urgent and ongoing medical concerns in the comfort of your own home.
Social care services
We can help with housing, employment, legal aid, food, childcare and other services you may need.
Mental health care
We provide a wide range of behavioral health services, including counseling, psychiatry, and integrated healthcare for substance use disorder and serious mental illness.
24/7 care line
Because your health doesn’t have business hours, you have a direct line to a provider when you need medical advice, 24/7.
Care coordination
We work with our members’ doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.
Complex care management
Our care teams are experienced at working with members who have chronic conditions and disabilities. Whatever matters to you–making it to the next family reunion, playing with your nieces, or just feeling more at peace–that’s what matters to us.
Urgent care that comes to you
We can be at your doorstep for non-life-threatening situations such as the flu, nausea, abdominal pain, chest pain, general pain, cuts, or burns.

Our care team will work with you to create a plan, and do what it takes for you to become your healthiest self.
Community Health Partners (CHPs)
Although you may see a team of providers with us, one thing will always remain the same—they’ll be available when you need them, and will never put a time limit on your care. They will also work with your current doctors, or if you have one that you trust, so there won’t be any gaps in your care.

*In-home urgent care services are provided where permitted by law and, as required, facilitated by and/or with advanced practice practitioners and/or physicians. Please call 911 for life threatening emergencies.
**Most of our services under Medicare/Medicaid Managed Care do not have copays. If there are any fees, we have a financial assistance program for eligible members.
