When you become a Cityblock member, our services are typically included in the benefits of your health plan. In the rare case where you have a copay, please reach out to us. If eligible, our financial assistance program may help cover your fees—making our services at no cost to you**.

No, most of our care can come to you, can be virtual, or over the phone. If you would like to visit us in person, we have hubs near you.

You can find information about your providers on your city page. You can always call us at 833-904-2273  24 hours a day, 7 days a week so we can introduce you to a provider before you decide to use them.

Yes. You can request translation services prior to an appointment by calling us at 833-904-2273 (TTY: 711) 24 hours a day, 7 days a week.

*In-home urgent care services are provided where permitted by law and, as required, facilitated by and/or with advanced practice practitioners and/or physicians. Please call 911 for life threatening emergencies.

**Most of our services under Medicare/Medicaid Managed Care do not have copays. If there are any fees, we have a financial assistance program for eligible members.
