We work hard to connect you to the right care as quickly as possible. If you’re having a crisis or another urgent situation, we provide rapid services.*

A team you can count on
All members of your Cityblock care team talk to each other and know your health needs. This means your care across providers will be consistent, and you won’t have to tell your story over and over. Your Cityblock team may also bring on licensed behavioral health specialists if you need it.
A bridge to long-term stability
It can be difficult to find the right mental health provider or community program. Cityblock focuses on filling in that gap with meaningful treatment. We’ll work with you as long as it takes to ensure you’re safely transitioned to the right kind of care for you
More than just referrals
When you need a referral, we don’t just send in the paperwork. We help you decide on the right provider and make sure they’re covered by your insurance. We have relationships with great providers, so when we refer you, it’s to someone we believe will give you excellent care.
Extra support and guidance
Your Cityblock care team, and especially your Community Health Partner, are here to support your mental health outside of the doctor’s office. We can help with scheduling appointments, setting up video calls, or just provide someone to talk to.
