Your Cityblock Community Health Partner (CHP) will talk with you about what’s important to you right now. Working together, you’ll make a plan for what’s next.
Your care team takes action
Your CHP will take real, concrete action towards getting you what you need. They’ll be upfront with you about the steps they’re taking and what you can expect to happen.

What’s a Community Health Partner?
And what can they do for you? Your Cityblock care team is assembled based on your needs. It can include doctors, nurses, pharmacists, mental health advocates, and social workers. But every Cityblock care team is driven by a Community Health Partner (CHP).
Here’s what to know about CHPs:
CHPs are your go-to for all things health–and beyond
Your CHP is like a supportive friend who happens to be really knowledgeable about the healthcare system. No question is off limits. They’ll never, ever judge you. They’re your number-one cheerleader, advocate, and tour guide through the healthcare system. But they’re also here to celebrate your triumphs, support your goals, or just talk about your day.
Your CHP knows your community
We hire Cityblock CHPs from within the communities they serve. They walk the same streets you do, and they’re plugged into community resources where you live.
A CHP means you’ll have a champion in your corner
Your Cityblock care team is always in close contact. They make sure your needs are being heard and addressed. Your CHP helps keep you and all your providers on the same page. When you have a new goal in life or don’t know who to call about your healthcare, reach out to your CHP.
