Whatever time it is, you can call Cityblock if you have physical, mental, or social care needs. We meet you on your terms, whether that’s at home, in your community, at one of our clinics, or by phone. We’re always here to listen and support you, however you need us.

On-demand video chats with a doctor.

Our doctors are ready to video chat with you to make sure you have the care you need.

A comprehensive care team.

You will be assigned a care team that includes a primary care doctor, nurse, behavioral health specialists, and a designated Community Health Partner to help you get the care and services you need.

Expert support for  complex needs

We work with your doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.

Pregnancy Support

For our members who are pregnant, expecting, or new parents, we provide 24/7 support, including doula support, to help with any prenatal and postpartum needs.
