When you become a Cityblock member, our services are typically included in the benefits of your health plan. In the rare case where you have a copay, please reach out to us. Our financial assistance program may help cover your fees—making our services at no cost to you.
We offer care for your whole self, including your physical and mental health. We also offer support for things that matter to you such as: getting you answers from a doctor quickly, rides to your appointments, childcare support, a care team that can come directly to your home, and more.
