Together, you and our care team will create an action plan to meet your goals. We coordinate with members’ existing providers, or assign physicians based on your specific needs.
24/7 care line
Because health doesn’t have business hours, members have a direct line to a provider when they need medical advice, 24/7.
Social services
We can help with housing, employment, legal aid, food, child care and other services members may need.
Behavioral health
We provide a wide range of behavioral health services for members, including counseling, psychiatry, and integrated healthcare for substance use disorder and serious mental illness.
Care coordination
We work with our members’ doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.
Complex care management
Our care teams are experienced at working with members who have chronic conditions, and disabilities.
Lab draws
Our care team can perform members’ blood work at home or at our local clinic.

While we don’t offer walk-in medical appointments at our clinics, members are always welcome to stop by to ask a question, charge their phone, connect to wifi, or get warm on a cold day. If you are a member in need of urgent care*, call our 24/7 care line at 833-904-2273 to speak directly with a licensed provider.
