Our Columbus location provides primary care, behavioral health, and additional supportive services for Ciytblock members. This location is inside the 4 Market Exchange Building in downtown Columbus. We’ll provide you with a dedicated care team that will do what it takes to get you the care you need.

We coordinate with members’ existing providers, or assign physicians based on your specific needs.
24/7 care line
Because health doesn’t have business hours, members have a direct line to a provider when they need medical advice, 24/7.
Social services
We can help with housing, employment, legal aid, food, child care and other services members may need.
Behavioral health
We provide a wide range of behavioral health services for members, including counseling, psychiatry, and integrated healthcare for substance use disorder and serious mental illness.
Care coordination
We work with our members’ doctors, specialists, and case managers to arrange home-based care, in-person appointments, and virtual support as needed.
Complex care management
Our care teams are experienced at working with members who have chronic conditions and disabilities. We help members reach the health goals that matter to them.
Lab draws
Our care team can perform members’ blood work at home or at our local clinic.

Our Columbus clinic offers primary care, in-home urgent care, behavioral health, social services, care coordination, complex care management, and lab draws for Cityblock members.
The Columbus location is open Saturday 9am to 5pm and only offers virtual services on Sunday. If you need assistance outside of weekend hours you can always call our 24/7 urgent care line at 833-904-2273 (TTY: 711).
