Look out for reminders

Bring your documents
Bring a photo ID so we can verify your identity—this could be your driver’s license, passport, or student ID. You’ll also want to bring your insurance card from your health plan.

What to expect during your visit
Fill out the essentials
Just like when you sign forms at a doctor’s office, we’re required to have you say “yes” to medical care. By signing a consent form, you protect your rights as a patient, your privacy, and your personal details.
You may have already signed these forms online. But if you haven’t, it’s okay to wait until your first appointment and go through them with your care team.
Make a plan
During your first visit, your care team will get to know you and learn what your daily life looks like. They’ll conduct a health assessment. Together, you’ll make a plan to help you set and reach health goals.
Take as much time as you need during this visit to share what’s important to you. Your care team is there to listen.

Community Health Partner (CHP)
Think of your CHP as your number-one ally. Your go-to for all things health. Lean on them to find housing and childcare support, get help managing prescriptions, set up video calls with your doctors, speak up for you, offer judgment-free support, and much more.
Medical Providers
Through Cityblock, you have access to doctors and experienced nurses who will diagnose and treat injuries or illnesses. And best of all, we’ll come to you. Call us, and we can be at your doorstep for non-life-threatening situations*.
Mental Health Providers
Your mental and emotional health is at the center of the care we offer. You have access to a range of behavioral health specialists for substance use disorder and serious mental illness.
Additional Team Members
Your health story is unique to you, and so is your care team. Depending on your needs, your full team can include dieticians, pharmacists, and pre- and postnatal experts. They coordinate with you and each other to make sure you’re getting the right care.
